Special notice: The Webelos Shooting Sports Twilight Camp at Token creek County Park has exceeded capacity and is closed to new reservations or roster submissions. We will accept requests to be put on a wait list. All Packs with reservations, Packs that have paid fees or Packs  on the wait list will be contacted after the April 1 early payment deadline regarding options available.

Health Related Forms:

  • BSA Health Form:  Beginning in 2010 the new BSA Health form will be required for all events. The new form replaces the old Class I, II and III BSA Health form which will no longer be accepted.  The length of continuous stay at a Scouting event determines which sections need to be completed.  The annual physical with a doctor's signature is required for all people participating in a long term camp (more than 72 hours).  Use the Link above.  Because of record keeping requirements from the State of Wisconsin we will need a copy of the Health Form that can be retained by the Glacier's Edge Council for each session of camp a Scout or adult attends.
  • Glacier's Edge Prescription Medication Form Everyone, both youth and adults, taking prescribed medications must have these listed on "GEC Camp Prescription Medications Forms."
  • GEC Religious Exemptiom from Medical Care & Treatment--Adult Form
  • GEC Religious Exemptiom from Medical Care & Treatment--Youth Form

Cub Scout Camp Registration Forms and Rosters

Use the roster form below to the list of youth and adults attending camp. The completed roster is to be submitted with payment of camp fees. The roster form is used for all Cub Scout camps. A separate roster must be submitted for each camp or camp session your pack is attending. For a pdf document with instructions for completing and submitting your Cub Scout and Webelos Camp rosters click here. This instruction document also includes information regarding when to turn in registration and health forms for each camp. The instruction document is included in the roster download. See the leader guides for additional information.

Use the registration forms to gather the information needed for the camp roster from the Scouts and adults attending camp with your Pack. For camps at Brigham, Carlson and Token Creek these registration forms should be turned in at the Madison Service Center with the camp roster and fee payment.  For Camps at Camp Indian Trails and Ed Bryant Scout Reservation the forms may be turned in on the first day of camp.

Reservation Deposit Forms - deposits for 2010 Cub Scout Summer camps will only be accepted until March 31. After that date Packs must submit a roster with Scout names and full payment to secure a place at a camp. To get a deposit form click here
